Report of Findings

Delivered by a Licensed CHEK Practitioner

Confidential and personal Wellness Screen summary

Client Overview

This report summarizes your Integrated Wellness Screen. It's a professional, non-invasive evaluation of your physical structure and lifestyle patterns. While not a medical diagnosis, it highlights key areas of imbalance and offers insight into how your body may be compensating, adapting, or struggling.

Purpose of the CHEK Integrated Wellness Screen™

If you're reading this, chances are you're not new to pain.

Maybe it's your back, hips, or neck. Maybe it's fatigue that never seems to go away. Or maybe it's the endless loop of short-term fixes – chiropractors, massages, medications – that provide relief for a while... until the problem returns.

This screen exists because you deserve more than symptom management. You deserve answers. And a path forward.

The CHEK Integrated Wellness Screen was created for people like you – individuals who are tired of masking the problem and are ready to understand what's really going on. Whether your pain started recently or has been with you for years, this process helps us uncover the underlying causes of dysfunction through a whole-body lens.

Chronic pain rarely begins where you feel it. And it never ends with a quick fix.

The human body is an integrated system. A misaligned pelvis can influence your neck. Shallow breathing can feed anxiety and inflammation. A gut issue can create back pain. A stressed nervous system can sabotage recovery. This screen gives us insight into these hidden relationships so we can begin to unwind them – strategically, not randomly.

Whether you're here for prevention or rehabilitation, this screen helps you:

  • Identify the stressors and imbalances that are driving your pain or symptoms
  • Connect the dots between your physical structure, core function, breath, lifestyle, and emotional health
  • Establish a clear picture of where your body is compensating – and what to do about it
  • Lay the foundation for a systematic, individualized plan that can reduce or eliminate pain for the long term

A note of Reassurance

Dysfunction is more common than you think. No body is perfect, and you are not broken. These results simply shine a light on how your body has adapted over time. As CHEK Practitioners, we don't just assess, we coach and mentor you through the journey back to real, lasting health. Our job is to identify what can be realigned and recalibrated – across your movement, lifestyle, and nutrition – to restore balance, reduce pain, and unlock the energy you need to live life to the fullest.

Whether you're a former athlete or someone who's always struggled with "healthy living," our process is designed to make this journey both effective and enjoyable. No matter your starting point, we'll meet you there, and help you discover a new level of strength, vitality, and happiness you may not have thought possible.

Detailed screening results

1. Postural Assessment

Implications:

Corrective focus:

Using the CHEK system, we combine targeted corrective exercise, myofascial release techniques, and postural awareness tools to restore natural symmetry, reduce strain, and support functional, pain-free movement.

Next steps:

To uncover the full story behind your posture – and begin building a truly personalised corrective program – we recommend completing a Full CHEK Assessment. This is widely regarded as the industry's most comprehensive assessment process, going far beyond surface-level observations.

In this full evaluation, we'll assess:

The result? A precise, science-backed corrective plan tailored to your exact body, health history, and long-term goals. If you've been wondering what's really holding you back – or how to unlock lasting freedom of movement – this is where we find the answers.

Spaces for Full CHEK Assessments are limited, and prioritised for clients committed to long-term transformation.

Please NOTE: We do not offer the assessments as a one-off service. Our assessments are part of the complete integrated package we offer.

2. Core Function & Stability

Implications:

Your "inner unit" – including the Transversus Abdominis (TrA), internal obliques, pelvic floor, diaphragm, and multifidus – is your body's built-in stabilisation system. It supports the spine, protects internal organs, and controls movement at its foundation.

When underactive, it forces the rest of the body to compensate – placing unnecessary load on the lower back, hips, knees, and feet.

Core dysfunction is often associated with postural collapse, pelvic misalignment, poor digestion, and chronic pain or injury.

If recruitment scores are below 50%, traditional strength training or gym exercises should be avoided. Without correcting the underlying control first, these movements can do more harm than good.

Breathing dysfunction and gut inflammation are also key players. An inflamed gut can neurologically inhibit core activation – making abdominal training ineffective until those stressors are resolved.

Corrective focus:

Core training at CHEK is not about crunches or reps – it's about retraining the brain to activate the body's stabilisation system with precision. This begins in the prone position, where we assess your ability to activate the TrA using a pressure cuff test. Our goal is always 100% recruitment in this position before progressing to more dynamic movements.

This approach is non-invasive, highly effective, and consistently delivers faster results than generic Pilates, fitness classes, or rehab programs. We build strength from the inside out.

But activation isn't everything. For lasting change, we also look at functional performance: Can you use your core when walking, lifting, squatting, or twisting? We assess how your system behaves under movement – not just in isolated tests.

Next Steps:

In your Full CHEK Assessment, we will conduct a comprehensive analysis of your entire core system, including:

  • Functional application of core engagement in everyday movements
  • Detection of scar tissue that may be disrupting core function – particularly from C-sections, hernia repairs, or abdominal surgeries like appendectomy
  • Neurological feedback loops that may need to be retrained due to trauma, inflammation, or inactivity
  • Deep integration of your breathing, postural alignment, and muscular timing

From this assessment, we will design a custom corrective strategy based on your exact alignment, physical history, and long-term goals – giving your core the attention it has likely never received, but has always needed.

3. Breathing Pattern Assessment

Implications:

Your breath is your body's most accessible – and often most underutilized – tool for regulating the nervous system. Shallow, chest-based breathing keeps your system in a state of fight-or-flight, increasing muscular tension, raising cortisol, and impairing digestion and recovery.

Breathing dysfunction contributes to:

  • Chronic stress and anxiety
  • Poor sleep quality
  • Reduced core activation and spinal stability
  • Fatigue and low energy
  • Unresolved pain and inflammation

Even more critically, breath is not an isolated function – it's affected by your posture, muscle balance, and even emotional holding patterns. Poor posture or muscular restrictions around the rib cage, spine, or abdomen can block full diaphragmatic movement.

That's why just joining a breathing class – without first identifying structural and functional limitations – often produces limited or temporary results.

Corrective focus:

We begin by restoring full, deep, diaphragmatic breathing. You'll learn how to reconnect to your breath in a grounded way that supports:

  • Nervous system regulation (shifting from stress to calm)
  • Better spinal support and core synergy
  • Improved digestion, circulation, and energy flow
  • Greater resilience to physical and emotional stressors

We teach simple but powerful techniques to reintegrate proper breathing into your daily life – techniques that create a true foundation for healing and performance.

Next steps:

In the Full CHEK Assessment, we carry out a detailed breathing analysis, which includes:

  • Testing for mechanical restrictions caused by posture or muscle imbalances
  • Identifying breathing patterns during movement and rest
  • Determining if past injuries, surgeries, or compensations are interfering with optimal mechanics

Only once these hidden blocks are uncovered can we truly create a breathing retraining strategy that works long-term.

4. Spinal & Joint Mobility

Observations: (Insert your 3 images below, marked with key findings)

Right side

Front

Implications:

Your spine is designed to move in all directions – forward, backward, side to side, and rotation. When movement becomes restricted in one area, other parts of your body compensate, often creating pain or dysfunction elsewhere.

Spinal restrictions can be caused by:

Limited spinal mobility doesn't just affect your back – it influences your entire kinetic chain, from your neck and shoulders down to your hips and feet.

Corrective focus:

We use targeted mobility work, corrective exercise, and manual therapy techniques to restore natural spinal curves and movement patterns. This includes:

Next steps:

In the Full CHEK Assessment, we conduct comprehensive spinal and joint analysis including:

This creates a complete picture of your movement system and allows us to design a precise corrective strategy.

Implications:

Your daily lifestyle choices are the foundation on which your body either thrives – or struggles. The patterns outlined in your questionnaire may seem small on their own, but together they form a powerful narrative about why symptoms like pain, fatigue, mood swings, and poor recovery may be persisting.

These imbalances contribute to:

These aren't just "wellness trends" – they are root-cause triggers we must address if your health is going to improve.

What you need to know:

There is no such thing as a one-size-fits-all diet or lifestyle protocol. What works for one person may completely fail another. Genetics, stress, digestion, metabolism, and activity levels all influence your body's ideal rhythm.

In fact, "following the rules" of generic nutrition often causes more frustration and burnout than it solves.

The truth? One-size-fits-all fits no one – at least not for long.

Corrective focus:

Your next step is to complete the CHEK Personalised Primal Pattern® Diet Assessment, a powerful tool that helps identify:

Alongside this, you'll complete several other core CHEK health questionnaires, which will give us deep insight into your internal stress load, organ health, and lifestyle rhythm.

Implementation strategy:

All of this information forms the foundation of your Personalised CHEK Health Plan, which is then:

This plan becomes part of your fully inclusive CHEK coaching program, which doesn't just give you advice – it walks with you through each change, every week, with full support.

Whether you're eating to reduce pain, boost energy, balance hormones, or improve performance – how and when you implement changes is just as important as what you eat.

Your next step:
From insight to transformation

The results of your CHEK Integrated Wellness Screen have likely revealed more than you expected. What may have begun as pain, stiffness, fatigue – or just the feeling that something wasn't quite right – is now beginning to make sense. You've seen how posture, breathing, core function, and lifestyle habits are deeply interconnected.

But this is only the beginning.

If you're here, it's because you're not looking for another temporary solution. You're ready for something deeper – something designed around you.

Why this matters

Most clients who reach this point have been through it all – appointments, treatments, short-term relief – but have never experienced a truly holistic, system-wide solution. The CHEK Integrated Wellness Screen gives us the map. What comes next is the plan.

The CHEK Approach:
Full assessment → personalised program

While the 30-minute screen provides a valuable snapshot, the Full CHEK Assessment goes significantly deeper. It's one of the most detailed and comprehensive health evaluations available in the field of integrative coaching and corrective exercise.

Your CHEK Practitioner will guide you through a full 2–3 hour assessment, covering:

From this, you'll receive a personalised written Report of Findings, which becomes the foundation for a fully tailored program – designed for your body, your goals, and your life.

The journey ahead

Most clients begin with a 3-, 6-, or 12-month program. Those seeking full transformation often work with their CHEK Practitioner for up to 12 months – not because it takes that long to feel better, but because true, lasting change takes time to build.

Your personalised integrated program will include:

This is your opportunity to move beyond symptoms – to reclaim strength, clarity, and confidence in your body and life.
